How to Create Waiver Questions

  1. Sign in to a PlayerFirst website.

  2. On the left navigation, click Program > Registration Details.

  3. If updating an existing program, select from the list and skip to step 5.

  4. If creating a new program, To the right of Search Programs, click .

    1. Type in the name of your new program.

    2. Click Create New Program.

    3. Select the program from the list.

  5. Click the Pencil icon.

  6. Click Waivers tab.

  7. Waiver Type: select Default or Custom from the dropdown menu (None is not applicable here).

    1. Custom allows the operator to modify the Default waiver language (ex: add an event or state-specific clause to the language, etc.).

  8. In the Create New Question section, input the following info:

    1. Question Name: Enter the question.

    2. Question Type: See Waiver Question Response Types.

    3. First Value: The first value the user will see.

      1. Ex: If the question is “What is your target level of competition for college?” and the answers are a) DI b) DII c) DIII d) Club e) Don’t want to play in college, the First Value would be a).

    4. Required: The default is to NOT require the question be answered. The box must be checked in order to require an answer.

    5. Ask When: Select the criteria for when the question will be asked.

      1. Always

      2. Tryout Registration

      3. Not a Tryout Registration

    6. Click Add Question.

  9. Repeat step 8 for any additional questions.

    1. Note: Remember the default is to NOT require the question be answered. The box must be checked in order to require an answer for each question.

  10. Click .
