How to Create a Player Evaluation Form

Sign in to a PlayerFirst website.
On the left navigation, click Training > Player Evaluation Settings.
Click Create New Eval.
Enter a Title for the evaluation.
Click Save Changes.
To add Evaluation Questions, type a number for in the text box to reflect the number of evaluation questions you’d like to add. Then click Add Questions..
When editing each Evaluation Question…
Order: The order of the questions.
Note: it is recommended to format the order at the end
Section: Specific sub-section of the evaluation.
Ex: Sections might be Offense, Defense, Technique, and Teamwork
Question: The specific skill being evaluated in a pre-defined category; not a free-response question
Ex: Technique is the question and Needs Improvement is the evaluation response; the evaluation response does not go into specifics
Note: The Evaluation Response is how a coach answers the question.
Note: Do not add new evaluation questions before clicking Save Changes to save questions that have already been updated. This may result in a loss of work.
Click Save Changes once all updates have been made.
Confirm Title of your evaluation is correct.
Confirm Order of the questions is correct.
Click Save Changes.
Repeat steps 6 - 11 until your evaluation is finalized.
To view the completed evaluation, on the left navigation, click Training > Player Evaluation Settings.
