1. Sign in to a PlayerFirst website.

2. On the left navigation, click Training > Player Evaluation Settings.

3. Click Create New Eval.

4. Enter Title.

5. Click Save Changes.

6. Click Add Questions.

   1. Order: the order of the questions

      1. Note: it is recommended to format the order at the end

   2. Section: specific sub-section of the evaluation

      1. Ex: sections might be Offense, Defense, Technique and Teamwork

   3. Question: specific skill being evaluated in a pre-defined category; not a free-response question

      1. Ex: Technique is the question and Needs Improvement is the evaluation response; the evaluation response would not go into specifics

7. Click Save Changes after each question.

8. Confirm Title is correct.

9. Confirm Order of the questions is correct.

10. Click Save Changes.

11. To view the completed evaluation, on the left navigation, click Training > Player Evaluation Settings.
